Milanovo village is located in the Vratsa Balkan, near the Iskar Gorge — an area with rich history, strong natural presence and deep cultural layers.

The first settlers in this area are migrants from the Rhodopes, who flee from the Ottoman invasions. They initially settle in an area known as Old Village, located about 30 minutes walk from today's Milanovo. It is there that the Old Church "Holy Mother of God" is still located, dating from the 14th century — testimony to the early spiritual and cultural presence in the area.

The settlers come from the Rhodope village of Osikovo, from which the original name of the settlement originates. At the beginning of the 20th century, the village was renamed to Milanovo, but the connection with the name Osikovo remains alive — both in the memory of the local people and in the history of the place.
